Polymorphed

You used to be a paladin. A warlock. A rogue with a reputation and a room at the inn under your own name. Something went wrong — nobody's totally sure what — and now you're a rat, or a raven, or, look, nobody's proud of the teacup thing, but here we are.

The quest didn't stop just because you did.

Polymorphed is a one-page RPG about adventuring in a body that isn't built for it anymore. One d6 pool, one rule, no HP, no GM rolls. Every risk you take is the same roll: commit some or all of your dice, any 4+ is a win, any 1-3 is gone for good. Enemies get one number — Toughness — and that's the whole prep a GM needs. What survived the curse with you is a single item, shrunk to your size, and it's yours no matter what the dice do to the rest of you.
